Montefiore Medical Center is the site of inpatient clinical activities of the Einstein Division of Geriatrics.

The Division of Geriatrics Acute Care Service provides comprehensive, integrated primary and consultative care for older adults hospitalized at Montefiore Medical Center. The service is comprised of community-dwelling older adults, residents from long-term and sub-acute care facilities, and consultations from surgical and other disciplines.

Primary care and consultations for older adults are provided at the patient’s and/or physician’s request. The Einstein-Montefiore geriatrics team provides medical care with a special focus on the following:

  1. maintaining physical and cognitive function in the hospital
  2. addressing decision-making and advance directives
  3. developing a discharge plan with the patient, family and caregivers

Einstein-Montefiore's Division of Geriatrics faculty physicians have particular expertise in the acute care of the frail elderly, with the goal of examining and improving the use of acute care services for this population. This highly visible service assists physicians in all specialties at Montefiore in managing elderly patients, as they are among the most vulnerable and complex patients admitted to the hospital.

The service is led by a geriatrician faculty member and staffed by geriatricians, geriatrics fellows, residents, and students, as well as nurses, pharmacists, rehabilitation specialists, and geropsychiatrists.

Bogaisky's Readmission Study Receives AAMC Honorable Mention
Research conducted by Dr. Michael Bogaisky initiated a unique collaboration to identify and resolve the issues leading to high hospital readmission among the skilled nursing facility patient population.
